Among phytoecdysteroids, a well-expressed hypoglycemic effect is known for turke sterone. Objective: We aimed to determine the effect of turke sterone on the endocrine and exocrine parameters of the pancreas in alloxan-induced diabetic rats. Material and Methods: White outbred rats were divided into 4 groups to receive saline or a single dose of alloxan (170 mg/kg, intraperitoneal) and/or turkesterone (10 mg/kg/24 h) and glibenclamide (5 mg/kg/24 h) for 10 days. For histological analysis the tail portion of the pancreas was kept in formalin for 3 days, dried in ethanol, parafinized and then stained with hematoxylin-eosin. The glucose, insulin, C-peptide and total protein level in serum, as well as α-amylase activity in serum and pancreas homogenate, were identified by using special kits (Human. Germany). Results: Diabetes caused complete destruction of the islet apparatus and partial destruction of the pancreatic acini tissue. Treatment with turkesterone of alloxan-induced diabetic rats resulted in restoration of the histostructure of endo- and exocrine pancreas, increase of serum glucose as well as decrease insulin, C-peptide, and total protein level. In addition, turkesterone treatment of alloxan-induced diabetic rats led to an approximation of α-amylase activity of pancreas and serum to control value. Conclusion: This study has shown that turkesterone led to improving the exocrine and endocrine function in alloxan-indused diabetic rats.Hence, it can be recommended for further testing as an anti-pancreatic and anti-diabetic drug in humans.

Assessing hand and foot dimensions for height estimation has always been of immense interest to the anatomist, scientist and forensic experts. The present study was carried out to determine the height from the foot length in the population of Saudi Arabia. Method: This observational cross-sectional study was conducted among male and female students aged 18-25 years studying at College of Medicine, Majmaah University, Kingdom of Saudi Arabia, having no foot deformity, spinal or bony injuries or surgeries. The obtained data were statistically analyzed. Results: The mean Stature(cm) of the male was 173.19±5.29, Left Foot length (cm) was 26.0±1.40)and Right Foot length (cm) was (26.0±1.52). While the mean stature (cm) of female was 158.56±5.55, Left Foot length (cm) was 23.19±1.14 and Right Foot length (cm) was23.19±1.16. Males having significantly higher values than females. The improvement of serum S100B levels is later found to be related to cerebral edema volume expansion. Methods: The prospective cross-sectional study involved patients with mild and moderate traumatic brain injury (based on the Glasgow Coma Scale (GCS)). Serum S100B protein levels were analyzed using the enzyme immunoassay technique. The relationship between serum S100B serum levels and cerebral edema volume expansion will be analyzed using the chi-square test. Results: A total of 24 patients involved in the research consist of 20 male patients (83.3 %) and 4 female patients (16.7%). The average S100B levels in patients are 0.07 07 ± 0.14 µg/L, and the average volume of cerebral edema is 16.6 ± 23.1 µg/L. There is no significant correlation of the level of serum S100B to the cerebral edema volume expansion (P = 0.45). Conclusion: The level of Serum S100B does not correlate to the cerebral edema volume expansion for patients with traumatic mild and moderate brain injury.

Stroke causes various disabilities, one of them is a sensory-motor deficit which will cause functional limitation one of them is STS which is significantly impaired. Stroke patients can experience many of problems with sit-to-stand activity due to muscular weakness, Inappropriate initiation, Muscle imbalance, spasticity, Inappropriate Weight distribution etc. Hence it is very important to rehabilitate the sit-to-stand task after stroke. Objectives: To develop a new lifting technique to overcome the drawbacks in the conventional method during sit to stand task. To find out the reliability and effect of this lifting technique. Methods: A total of 20 subjects affected with stroke were selected by Consecutive sampling and allocated randomly in both the groups (group A and B). Group A received Conventional lifting technique of sit to stand and group B received New lifting technique. During these techniques’ subjects were analysed with surface EMG in that Mean MUAP was noted. Results: Results showed extremely significant differences in Patients (Quadriceps & Significant difference in Hamstrings Muscle). In Therapist Biceps & Triceps showed Extremely Significant Difference. Conclusion: The new lifting technique which is based on kinesiological principles was found to have a significant effect on muscular activation which is needed during sit to stand activity than conventional lifting technique in stroke patients.

As a result, white patches appear in the body. Vitiligo may be segmental or non-segmental. Objectives of this review article is to understand the underlying mechanism of occurrence of vitiligo, brief an updated information about the available treatments and the possible precautionary measures against vitiligo. All available literature about vitiligo and it’s treatment procedures were studied and thus all available information has been reviewed and briefed in this article to represent a compact scenario of the pathogenesis and treatments of vitiligo to make the understanding of the disease clear. Vitiligo has significant social and psychological impacts and physiological adverse effects like hypopigmentation or depigmentation in the patient. The social and psychological impact that the disease brings with it is severe in most cases. Vitiligo is not completely curable but treatments include topical corticosteroids, immune-modulators, and UV therapy which causes repigmentation of white patches, micro pigmentation etc.

Several modifications come across but injectable platelet-rich fibrin (i-PRF) showed unique properties. The objective of all this innovation is to find out all the components from the autologous blood to elucidate healing & application in tissue engineering. These i-PRF has achieved predictable and effective results. The main difference of i-PRF from solid PRF is the lower speed and time in centrifugation for i-PRF. i-PRF is the liquid variety of PRF which may accelerate the wound-healing processes with increased vascularization. The benefits of i-PRF show slow and sustained release of growth factors, by releasing the expression of transforming growth factor-β and collagen-1 mRNA along with cells migration. The use of platelet aggregates in injectable form is commonly used in orthopaedics and plastic surgeries. Because it is autogenous, it decreases the chances of adverse reactions to the implanted material as compared to other ways of grafting which facilitate better option in regenerative procedures. It has been observed that i-PRF found to be effective in periodontal wound healing and bone regeneration. This review article focuses on the current status of i-PRF formulation advantages and uses in regenerative surgery along with the healing process.

The survey involved only female nurses which were provided with some questionnaires comprising some basic data of their diets during their pregnancy times. A journal concerning the same survey was published in the British Medical Journal in 2016 describing the whole process and significance of this survey. A rapid increase in the glucose level observed in pregnant women who consumed more potatoes in their pre-pregnancy phase. Women with higher potato consumption were examined to have increased blood sugar levels than those with lesser potato consumptions. It can be inferred from this study that women should avoid consuming more potatoes in their pre-pregnancy period, else; they may develop Gestational Diabetes Mellitus during their pregnancy which can affect both the parent and the baby.

The diarylquinoline derivative drug Bedaquiline which inhibits ubiquitous enzyme ATP synthase was introduced in 2012. ATP synthase is an essential enzyme that helps in supplying energy to Mycobacterium tuberculosis even in the unfavourable conditions which helps them to survive in macrophages of human beings. Since 2017, resistance to bedaquiline has been reported worldwide which emphases us for the discovery of the new drugs. The medicinal plant Tinospora cordifolia is used in various traditional healing systems worldwide to treat diabetes, leprosy, etc. Methods: The current study summarizes the pharmacology of the phytocompounds of Tinospora cordifolia. In silico study was performed by using tools like CASTp, PubChem, ChemSpider, Swiss ADME, Pro Tox server, HEX dock, PyMol, and Lig plot+. ADMET study of the phytocompounds was also studied. Results: The results obtained in the study indicated that the 2-Hydroxy-3-oxoicosanoate was the potent inhibitor which shows the highest affection towards ATP synthase of Mycobacterium tuberculosis and also accomplishes the entire criterion to be an effective drug. Conclusions: Many of the phytocompounds of Tinospora cordifolia shows good ADMET analysis and docking result. Among all the phytocompounds 2-Hydroxy-3-oxoicosanoate shows the best result in terms of docking. This is only an in-silico study to fasten the drug discovery process, further validation of the proposed drug requires in-vitro and in-vivo study.

EC is used after the unprotected sex, unlike other regular methods of contraception which are taken before the sexual act. Appropriate knowledge of EC among health care professionals is required as EC are the only method that can be used to prevent pregnancy after unprotected sex or contraceptive accident. Aims and Objectives: To assess knowledge, attitude and practices regarding emergency contraceptive pills among Nursing students. Methodology: A cross-sectional study was conducted on Nursing students. Nursing students of all four years who were present and given consent on the day of data collection were recruited for the study. A pre-designed pre-tested self-administered questionnaire was used to assess the knowledge and attitude practices regarding emergency contraceptives among nursing students. Data thus collected were analyzed using appropriate statistical methods. Results: Out of a total of 297 nursing students who participated, 90% have heard about emergency contraceptives. Very few were aware of the various methods available for emergency contraception. Only 35.4% knew about the correct timings of their use. The majority had the misconception that EC can be used to induce abortion and as a birth control measure. 67.4% showed a favourable attitude towards advising a woman to use EC when needed. 24.8% said that EC was advised by them to patients or relatives. Conclusion: Although the awareness about emergency contraceptives is high but the knowledge regarding correct timings and usage is poor.

However, it gives a short lifetime of the specimen and they are more proven for fungus growth. We use high concentration formalin (30%) to achieve prior purposes and to observe any changes in the human tissues. The present study aims to compare high and low concentration formalin effect on the body tissues and rate of fungal growth. Methods: The donated bodies were embalmed with high and low concentration formalin in the right femoral artery. Then observation was made over the skin, muscle, nerves, vessels, and solid organs like liver, lung & brain. Results: In high concentration formalin the lifetime of the specimen was more but the quality and fungal growth were the same as low concentration formalin; the quality of the cadaver was poor compared to low concentration formalin. Conclusion: The use of low concentration formalin results in good quality cadaver over the high concentration formalin.

Overall, this condition is a threat to the life of the patients, hence to improve in the patient’s condition dialysis and renal replacement therapy is a must. But before the onset of dialysis and renal replacement therapy, here, early vaccination becomes must as a primary treatment modalities to prevent patients from nosocomial infections i.e. hospital-acquired infection like Hepatitis-B, Influenza, Pneumococcal diseases, these are the common diseases due to such infection and their prevalence rate is high in India as compared to others diseases. Therefore, to prevent the patients from any such risk early vaccination is needed. This review aimed to access this information for educating the patients regarding the importance of vaccination before the onset of dialysis and renal transplantation, to suggest that this vaccine should be available at various private and government hospitals at low prices than its original price and to evaluate the effectiveness of the awareness program on vaccinations among chronic kidney disease patients. Here the problem is that many patients belonging to lower socioeconomic status can not afford this costly vaccine and many patients are not aware of this vaccination needed in chronic kidney disease before starting of dialysis, because of lack of education or no any such information received from health care professionals. Hence it’s essential to create awareness regarding vaccination among chronic kidney disease patients to reduce morbidity and mortality.

Gallbladder disease is the second most common cause of non-obstetrics surgery after appendicectomy. Aim and Objectives: In this case series, we are trying the description of the various presentation of gallstone and its related complication during pregnancy and the postpartum period. Methods: The reported documents of 6 patients with complications of cholelithiasis during and after pregnancy, was collected after from hospital database from period 2016-2019. Results: In these 6 cases, 2 patients presented with acute cholecystitis at 30 and 15 weeks of gestation. Both of them were managed conservatively with antibiotics. The cholecystitis resolved and interval cholecystectomy was performed after delivery. Another 2 patients presented with acute cholecystitis during the 1st month of the post-partum period. These 2 patients were treated conservatively, and interval cholecystectomy was done after 6 weeks. Two patients with obstructive jaundice underwent endoscopic retrograde cholangiopancreatography (ERCP) and undergo interval cholecystectomy. Off 1 patient encountered post-ERCP complication. Despite complication ERCP followed by laparoscopic cholecystectomy is well available modality for common bile duct (CBD) stone with obstructive jaundice. Conservative management is the best way of treatment to avoid maternal and fetal morbidity and mortality. Conclusion: The conservative management of cholecystitis is a better way to treat during or immediately after pregnancy and interval cholecystectomy is a better line of management for cholecystitis pregnant patients. Although ERCP is having its complications, it is one of the best modality of extraction of CBD stones and followed by interval cholecystectomy later.

The stress involved in meeting the responsibilities of becoming a physician may adversely affect the exercise and dietary habits of students. Objectives: To assess the practice of physical activity and dietary habits amongMBBS students of a private Medical College of Eastern India. Materials and Methods: A cross-sectional study was conducted from July-September 2018 among 130 MBBS students a private Medical College of Eastern India. Physical activity was assessed by using the Global Physical Activity Questionnaire (GPAQ) and dietary habits by a pretested and pre-structured questionnaire. Data were entered into an excel spreadsheet and analysed by SPSS software version 20. Results: Mean age of study participants was 21.99±0.96 years. About 55.4% were females, 93.1% were Hindus and 92.3% belonged to general caste. According to WHO criteria for obesity, 28.5% were overweight and 11.5% were obese. The median total energy expenditure was 599.01 metabolic equivalent-min/week (IQR-420-780). Normal-weight/ underweight students were more likely to be achieving recommended physical activity (PA) levels than overweight/ obese participants based on overall MET value and this difference was statistically significant (p=0.003).40.8 % of students were not physically active. About 19% were not involved in any activity at leisure, 61.2% thought they were overweight. Among them, 65.4 % practised various exercise to maintain a healthy weight. Conclusion: Promotion of physical activity by increasing awareness and specific intervention for obese students is needed for obesity control.

One such modality is the use of Hydroxychloroquine/Chloroquine. The objective of the survey was to understand the awareness and impact of HCQ/CQ prophylaxis among the health care workers (HCWs) especially anaesthetists as they are fighting it on the frontline. Methods: A web-based, cross-sectional survey was conducted for HCWs globally. Participation was voluntary and confidentiality was maintained by making participants’ information anonymous. The questionnaire consisted of 28 items. Data were tabulated in excel, and descriptive statistics were performed. Results: The survey was taken by 344 HCWs from all over the world. 98% of participants heard about the use of HCQ/CQ prophylaxis against COVID 19 infection. 301 HCWs knew about the side effects of HCQ/CQ. 54.1% of participants agree there is not adequate research done. 122 participants took HCQ/CQ prophylaxis. Out of 29.5% participants who received the medicine from the hospital under hospital protocol, 66.7% were given medication without baseline investigations and 30.5% HCWs were not even briefed about the drug and its side effects by the hospitals. 36.2% of participants developed side effects. 8.7% of HCWs were tested for COVID19 out of 344 participants. Conclusion: The drug taken by HCWs was without adequate evidence, prior investigations, supervision and follow-up. Most of the participants self-prescribed the drug. No separate guidelines were stated for people who had co-morbid conditions. Hospitals neither conducted baseline investigations and nor briefed HCWs about HCQ/CQ. These are some serious concerns we are looking into as who will be answerable in case of adverse events.

Aims and Objectives: The purpose of this study was to detect Mycobacterium tuberculosis (MTB) in clinically suspected cases of pulmonary tuberculosis by using staining methods. Methods: A prospective examination of samples were performed in the department of microbiology in Government Medical College, Amritsar. During this study, a total of 100 sputum samples from patients clinically suspected of pulmonary tuberculosis were collected in sterile leak-proof containers as per RNTCP guidelines. The collected samples were divided into the following three parts: initial part was processed before decontamination; Second part processed post-NALC-NaOH method and third post Bleach method and then subjected to ZN, Kinyoun and Fluorescent staining. Results: Among the following methods, the detection rate of acid-fast bacilli (AFB) by prior decontamination was more with Fluorescent (27%), followed by Ziehl Neelson(ZN) ( 25%) and then by Kinyoun staining (20%). Furthermore, post decontamination, the detection rate of AFB by post-NALC-NaOH method increased as the fluorescent method (37%), ZN (30%) and Kinyoun method (23%). Whereas the detection rate of AFB by post bleach method was less effective i.e. fluorescent staining (32%), ZN (26%), and Kinyoun (20%). Conclusion: The rate of detection and efficiency of acid-fast bacilli was found to be more with Flourescent staining. This research also highlighted the increase in the detection rate of AFB after decontamination with NALC-NaOH method as compared to the bleach method thus it aided in increasing detection of bacilli.

Purpose: This research is a patient management system which is called MedCo can solve this problem by enabling the patient’s personal information and brief medical records are accessible to the patient, paramedic and all users who are using the system by scanning the patient’s fingerprint. Conclusion: This can be used for registration at the registration counter as well as it can save a lot of time. The staff can scan the patient’s fingerprint to access the patient’s personal information and send it to the queue system. This system can be developed by using Dart programming language through Flutter for cross-platform development. This system will be build using Visual Studio Code and it requires the use of local_auth libraries to code for biometric recognition. Firebase and Google chrome will be used in the development process. This system will be developed for the mobile user that can support Android and iOS while for the desktop user, web applications will be developed.

Though regression is usually used for both purposes, the way within which the model is built will differ thanks to the goals of the model. The goal of a prediction model differs in several important ways. Mathods: Using a cohort design that links baseline risk factors to a validated population-based diabetes registry, a model (Diabetes Population Risk Tool or DPoRT) to predict risk factors for diabetes using commonly-collected national survey data was developed and validated. The event cohort was the National Population Health Survey (NPHS) linked to the validated Diabetes Database, a provincial component of the National Diabetes closed-circuit television (NDSS). Variables were restricted to factors routinely measured within the population. The probability of developing diabetes was modelled using sex-specific survival functions for those > 20 years, without diabetes and not pregnant at baseline (N = 19,000). Results: The age-standardized 5-year incidence rates in the development cohorts were 6.52 % for males and 5.42 % for females. The 3-year age-standardized incidence rates in the development cohort were 3.42 % for males and 2.41% for females. The age-standardized 5-year incidence rates in the development cohorts were 6.42 % for males and 4.20 % for females. The age-standardized 3-year incidence rates for validation cohort was 3.45 % for males and 3.22 % for females. Conclusion: Determinants of weight and weight change are essential when developing strategies to prevent or reduce the future diabetes burden. In monitoring trends over time researchers are often faced with the dilemma of separating trends between individuals and trends within individuals. Multilevel growth models allow us to model both these aspects which strengthen the ability to model trends that vary between and within individuals.

Globally, more than one-third of the world population is infected with the bacteria that cause tuberculosis (TB) and each year approximately 9 million people affected with the disease and each year 2 million of those die. Aim: To synthesize a series of novel 3,4-dihydropyrimidine chalcones which have anti-mycobacterial activity and to perform docking studies for active compounds. Methods: A series of 6-methyl-4-phenyl-3,4-dihydropyrimidin-2(1H)-one derivatives were synthesized and evaluated their antimycobacterial activity against M. tuberculosis H37Rv strain using Microplate Alamar Blue dye Assay (MABA). The docking calculations were done on protein, thymidylate kinase (PDB ID: 1G3U) employing in Genetic Optimization for Ligand Docking (GOLD v4.0.1 2008) software using Genetic algorithm. Results: Among all the synthesized derivatives, 5-(3-(2-thienyl) acryloyl)-6-methyl-4-phenyl-3, 4-dihydropyrimidin-2(1H)-one was exhibited potent activity compared with reference standards pyrazinamide and streptomycin. The potent compound forms H-bonding with Tyrosine39, Glutamic acid 166 and Arginine160 amino acids of the active site, a 2-thienaldehyde derivative which undoubtedly accounts for their superior activity associated with other analogs of the series. The docking results exposed useful information to understand the interaction mode between 3,4-dihydropyrimidine chalcone derivatives and thymidylate kinase will simplify the next cycle of drug design to reconnoitre the newer lead molecules. Conclusion: The interaction model between dihydropyrimidine chalcone derivatives and thymidylate kinase will facilitate the next cycle of drug design to explore the newer lead molecules.

Objective: Determination of the need for private medical services and state regulation in the event of a pandemic. Results: The article emphasizes the expediency to maintain the partial smoothness of these services, taking into account the fact that the introduction of market laws in the provision of medical services and the provision of paid services as a whole can lead to dangerous consequences and cause social problems, adding a system of paid infertility. In the introduction of market legislation to medical services, the expediency of employment of the population to be studied in such aspects as their monetary income and cultural level how positive these aspects are will be brought about in such a way that it will reduce the negative impact of the medical services market practice to such an extent. The state avoids the problems between workers and subjects in the social sphere. All this is explained by the emergence of methods of harmonization of the social sphere and states with the market mechanism of regulating its development in a single framework. Conclusion: In the conclusion of the scientific work, in the current conditions in which the coronavirus pandemic is widespread, proposals have been made on alternative methods and directions of the emergence and regulation of the population as the main reformer of the state in the equal use of special medical services among state medical institutions. The reforms carried out in Uzbekistan in health care and their social orientation, equality in the use of medical services have been preserved, and necessary proposals have been made to regulate private medical services by the state.

Detailed analysis of Naga Parpam was done which shows Mg, Si and K as dopants. Photoluminescence study was also conducted which shows a peak at 649 nm. Aim and Objective: It is important to analyze due to which dopant this has occurred and hence two separate studies, Mg-doped ZnO and SiO2 doped ZnO were conducted. PL studies done on SiO2 doped ZnO shows significant peaks at 380nm and 649nm. Further, PL studies on Mg-doped ZnO shows a small peak at 381nm and two significant peaks at 640nm and 648nm. Result: Though the peak at 380nm is the characteristic peak for ZnO, when it is doped with Mg, the PL shows only a small peak. This shows the structural implications of the dopant and Match 3. XRD analysis showed secondary peaks for Naga Parpam and Mg-doped ZnO but not for SiO2 . Match 3 analysis showed SiO2 doped ZnO and ZnO has a hexagonal structure and Si has a cubic structure. In Naga Parpam, ZnO and Mg have a hexagonal while K and Si has a cubic structure. In the case of Mg-doped ZnO, both ZnO and Mg has a hexagonal structure. Hence in Si-doped ZnO, though there are two crystal systems hexagonal one and the other cubic, there are no secondary peaks observed in XRD, which has been established only from Match 3 analysis. But, in Mg-doped ZnO, it is observed that both ZnO and Mg has a hexagonal structure, from Match 3 analysis. PL analysis shows two sharp peaks at 640 nm and 648 nm and the characteristic peak of ZnO, which is the main constituent of the material is suppressed. Conclusion: It is concluded that when the dopant changes its characteristic structure to that of the bulk material, in PL analysis, the bulk material characteristics are suppressed. This may be the mechanism for mimicking the molecule and hence, this analysis may be a potential contender for drug delivery.

Aims: To explore socio-demographic characteristics, associated depression, prior help-seeking, and beliefs regarding causation and consequences in patients with Dhat syndrome. Method: Prospective study on 100 patients of dhat syndrome attending Psychiatry department as an outpatient in a tertiary care general hospital in western India. Depression was assessed by the Patient Health Questionnaire (PHQ 9). Patients were interviewed for beliefs about dhat, prior help-seeking, and explanations of dhat offered by the helpers. Patients’ views why they have dhat syndrome and opinions about the consequences of dhat syndrome were explored. Results: The majority were the young adult, migrant workers from neighbouring states with education up to high school, and belonging to lower socioeconomic status. Depression was found in 38 % case. Patients reported a significant decline in sexual functioning after the onset of dhat- 18 had premature ejaculation, 28 had erectile dysfunction and 20 had small penis size as associated complaints. Most patients had consulted non-psychiatry ‘experts’- quacks, general practitioners or practitioners of Ayurved/homoeopathy/ Unani, and dermatologist-venereologist. Most patients believed that dhat can cause somatic symptoms are commonly seen in depression as well as sexual dysfunctions. Conclusion: The study largely confirms the findings of earlier studies on dhat syndrome. A significant proportion of patients are having depression and decline in sexual satisfaction and sexual dysfunctions. They also believe sexual myths often propagated by quacks.

Management options include observation in the initial stages, and LASER and anti-vascular endothelial growth factors (antiVEGF) injection in advanced cases. The symptom of blurred vision may be correlated with Timira, a Drishtigata Roga (disease of vision) according to Ayurveda. Case Presentation: A 48-year-old male presented with the blurring of vision, floaters, and curved vision. Management was on an outpatient basis over 3 years, during which oral medicines, ophthalmic drops, and external application of paste were administered. Results: Improvement was noted in visual acuity. Optical coherence tomography (OCT) macula showed gradual absorption of sub-retinal fluid. Conclusion: The main aim of management was to restore vision as much as possible while giving the patient a better quality of life. This study illustrates that cases of CSCR can be successfully managed using Ayurveda, and these treatments can be considered as an alternative option.

Persistent pain, oedema and broadening of the heel are common complications postoperatively. The treatment is controversial and none of it is ideal. Results of fixation with the calcaneal plate are not satisfactory and other surgical treatments can’t prevent subtalar collapse. Methods: Comminuted, complex, depressed fracture of the calcaneum is elevated with Steinman pin insertion which is used as a joystick. If necessary, the cortico-cancellous screw is pushed to support the subtalar joint. The vacuum is created by elevation within the shell of the calcaneum which is pierced by the guidewire and then the DHS reamer is passed from posterior to the anterior portion of the calcaneum to shift the cancellous portion of the bone in the desired area. The harvested fibular stud graft is pushed from posterior to the anterior aspect of calcaneum over a guidewire to support the gap. So the fibula acts as a supporting stud as well as the cortical cancellous graft. Results: The criteria for patient-related outcome measures include a reduction in pain, correction of the broadened heel, tiptoes and heel touch standing, painless walking and radiological and clinical healing of the fracture, correction of subtalar depression. Out of twenty-seven patients, we have achieved excellent results in twenty-six patients and good result in one patient. Conclusion: This is our innovative minimally invasive technique for tackling comminuted complex depressed fractures of the calcaneum. Fibular stud graft helps in packing the gap, providing stud support as well as cancellous grafting which is giving excellent results of maintaining the heel contour of a proper size, relieving the pain early and providing a virtually good range of movement.

Carrot is an easily available vegetable and is utilized for various dessert dishes. Therefore, the present study was conducted for the optimization of the process for the development of carrot supplemented paneer kheer. Methods: Four different levels of paneer and three different levels of carrot was used for the formulation of carrot supplemented paneer kheer. i.e., 100:0 (T0), 80:20g (T1), 60:40 (T2) and 40:60 (T3).Optimization was done based on results obtained from physic-chemical analysis and sensory evaluation. CRD was utilized for experimentation and the significance was shown according to the critical difference at the rate of 5%. Results: T1 (80:20) was selected as the optimized product after the analysis. The optimized product contained moisture (45.31%), protein (24.21%), ash (3.42%), pH (6.16) and titratable acidity (0.42%). Overall acceptability in terms of the sensory evaluation was found to be highest for T1 (8.5) then, for T0 (8.3) and T2 (6.61). T3 (5.14) was neither liked nor disliked due to the higher proportion of carrot paste. Conclusion: The composition of T1 was found to have a balanced proximate composition as well as acceptable sensory characteristics; which shows that paneer kheer could be enriched with nutrients of carrot and enhance the palate of the consumers.

It may further result in gingivitis and periodontitis. Majority of the persons affected with plaque may not give much attention unless they experience some other symptoms like gingival bleeding, halitosis, dryness of mouth and sensitivity. Dental plaque may be compared with Dantasharkara, one of the dantaroga according to Ayurveda. Aim: For the case series under reference, Dantasharkara Uddharana (removal of plaque) with shastra is told in Ayurveda. But the procedure is not standardized. So, the second line of treatment Kshara and Madhu pratisarana [rubbing] is adopted here. Case Report: In the present clinical study, 30 patients were registered to have a complaint of Dantasharkara. Each case was observed for prevalence according to age, gender, socioeconomic status, occupation, diet pattern, frequency of cleaning teeth, tooth discolouration, tooth sensitivity, halitosis, dryness of mouth, plaque index and gingival bleeding index. Result: 250 mg Saptachada kshara pratisarana and 100ml Tilayastimadhu ksheera gandusha daily once for 30 days showed significant reductions in tooth discolouration, sensitivity, halitosis, dryness of mouth, plaque and gingival bleeding indices, with an improvement of 83%,100%, 93%, 97%,95% and 95% respectively. Statistical analyses were performed with SPSS software and are clinically significant with p < 0.0001. Conclusion: Saptachada being a pratisaraniya kshara dravya is mentioned in kushtaghna mahakashaya and sa krimya Vrana chikitsa. Due to lekhana, chedana, bhedana pachana, vilayana and kapha vata shamaka karma, kshara becomes responsible for samprapti vighatana of danta sharkara. Saptachada kshara pratisarana is safe, effective and has proved to have better patient’s tolerance.

Dental casts were collected retrospectively from Riyadh dental Hospital. Methods: Human bite marks were registered on different materials like pink wax, aluwax and Polyvinyl siloxane. These study casts with their corresponding bite marks were imaged and categorized into: (1) 2D imaging using digital cameras following the American Board of Forensic Odontology guidelines; (2) 2D CBCT imaging with Cone-beam computed tomography (CBCT); and (3) 3D imaging using CAD/CAM and CBCT. Sensitivity, specificity and receiver operating characteristics (ROC) curves were traced for observers in all the approach. Paired t-test and kapa tests were utilized for the inter-observer consistency. Each modality had a significant difference under the ROC plotted. Results: The study observed that 3D CBCT imaging in comparison with the other imaging modalities had a marginally higher value of P= 0.0002. The kappa method for the inter-observer agreement was used to confirm this observation. Conclusion: This study clearly shows the advantage of 3D imaging using CBCT in human bite mark imaging. Based on the results, a 3D forensic odontological image database collection with a gold standard is the future potential of this study.

We investigated the association of serum OC levels with JIA and the influence of T1DM on this association in participants. Our goal is to study, analyze, and update and introduce new therapeutic techniques into the underlying disease characteristics of chronic rheumatic diseases that are most commonly observed in childhood. Methods: This study compares concentrations of osteocalcin (OC) between participants with PJIA, SJIA and type 1 diabetes (T1DM) (n=20) and age-, gender- and body mass index (BMI)-matched participants without T1DM (n=40) among patients with OJIA (oligoarthritic the juvenile idiopathic arthritis), and it explores relationships between OC concentrations JIA and T1DM. Quantitative evaluation of OC was measured in heparin-treated blood plasma by IMMULITE 2000 analyzers. This in vitro study used to monitor mineral metabolism and diagnose osteoporosis. The IMMULITE OC test allows determining only an unfragmented OC molecule, not fragmented ones. Results: Concentrations of OC were very low in participants with PJIA, SJIA with the combination of T1DM. In OJIA patients without T1DM, concentrations of OC were roughly at normal average. Conclusions: In patients with PJIA and SJIA, lower OC concentrations were related to T1DM.OC was sufficient to predict and prevent osteoporosis in JIA patients with a combination of T1DM. Serum level of OC administered in patients with OJIA without T1DM was in the region of the normal range. No relationship was detected between serum OC levels and OJIA in participants.

Females have long been supposed to have stronger immunity against all diseases including the viruses and nCoV-19 seems to be part of a larger spectrum. The causes of less prevalence and mortality rates in females could range from a multitude of factors like stronger innate immunity to sex chromosomes to steroid hormones. Females under being diploid X chromosome undergo XCI which leads to varied manifestations of many immunomodulatory genes. The lower prevalence of COVID-19 in females could be attributable to lesser ACE2 receptors, biallelic expression of TLR7, Ddx3x, CD40L and CXCR3, higher IFN’s, exaggerated expression of NEMO, IRAK1, JAK-STAT pathway, the higher number of macrophages, enhanced Th1 response and higher circulating antibodies in females. The major causes of death in a COVID-19 case has been reported to be ARDS, hypokalemia, respiratory failure, and multiorgan failure due to immune dysfunction. The increased expression of IL-6, CXCL-10, MCP-1, MIP-1α, CCL-14, and CCL-23 appear to tilt the balance in males towards an increased N/L Ratio leading to cytokine storm and ACE2 down-regulation progressing to hypokalemia, hypotensive shock and ARDS. Apart from the various protective effects existing in females in preventing the above complications, there is an upregulation of P53 and FOXP3 gene and a pro-apoptotic response which increases the Treg cells trying to bring back the female body to homeostasis.

This condition can be managed conservatively and that with nutrition hence avoiding major surgical intervention. A previously fit 13-year-old boy presented with non-bilious vomiting that had turned bilious, over 5 days makingsub-acute intestinal obstruction likely. Barium meal and follow-through were conclusive of superior mesenteric artery syndrome (SMAS) which was confirmed with a CT abdomen. He was managed with parenteral nutrition and positioning when taking oral feeds. He made a full recovery and demonstrated weight gain and continued to remain well without vomiting on follow up. Although a rare condition, SMAS should be considered in children with acute vomiting which can be managed conservatively as highlighted by this case without any known precipitating causes. The medical condition of SMAS is a known yet underused treatment which may avoid major intraabdominal surgery. In cases such as our parenteral nutrition was used as a rescue treatment and enteral nutrition followed recovery and convalescence.

In many hospitals, the physiotherapist manages different orthopaedic, neurological, cardiothoracic conditions. Previous studies suggest that intensity of physiotherapy reduce ICU stay and improve functional status. Although medical and nursing professions have provided 24-hour care, however great variability reported in aggregate sessions out of regular sessions given by physiotherapist. Methods: 100 clinically diagnosed with Moderate head injury subjects have participated in this study. Each individual was thoroughly examined and assessed through neurological assessment. These subjects were allocated in 2 groups (Intensive Physiotherapy group 3 physiotherapy sessions were given per day and Conventional group 2 physiotherapy sessions were given per day for five days per week for two consecutive weeks). Before and after the treatment protocol subjects were assessed for Consciousness, cognition and functional mobility by Glass Gow Coma scale, Mini-Mental State Examination and Chelsea Critical Care Physical Assessment Tool respectively. Rresult: Pre and post-treatment protocol was analyzed by GCS, MME and CPAx using paired and unpaired t-test.
