There are two models of ALPS namely generational and steady-state with their own merits and demerits. Present work has been taken up to devise a search algorithm E-Hybrid-ALPS with the combined concepts and advantages of both the models. Methodology: E-Hybrid-ALPS not only combined the concepts and advantages of both the models but also considered weak individual solutions to the mating pool and adaptively applied the crossover operator. A search algorithm, a component of the molecular docking tool plays a vital role in the success of molecular docking used in drug discovery. Hence, E-Hybrid-ALPS has been implemented as a search algorithm for molecular docking. The execution was carried out with two receptor-ligand combinations namely receptor CYP2C8 and ligand Chloroquine, a therapeutic option in the treatment of Corona Virus Disease (COVID-19) and also a drug used in the treatment of Malaria and receptor CYP2B6 and ligand Cyclophosphamide a drug used in the treatment of cancer. Results: E-Hybrid-ALPS generates poses of the ligand in the active site of the receptor, calculates the binding energy of each pose and outputs the pose with the lowest binding energy. The performance was evaluated by comparing it with the widely used molecular docking tools AutoDock and AutoDockVina which employ Lamarckian GA as a search algorithm. Lowest binding energy found by E-Hybrid-ALPS was significantly low as compared to the lowest binding energy found by AutoDock and AutoDockVina Conclusion: E-Hybrid-ALPS which generates a ligand/drug pose with the lowest binding energy can be implemented as a search algorithm for AutoDock molecular docking tool. This helps the drug discoverer in designing a drug with a better binding affinity as lower binding energies indicate higher binding affinity. Many medical schools curbed clinical interaction and replaced in-person classes with online methods. Baring a few review articles, research articles in this domain are scare. This study was done in the present context to understand the perspectives of the medical students on the different teaching methods adopted and the acceptability of those mechanisms from the view of the end-users is novel. Materials and Methods: This study was undertaken between 1 May 2020 to 15 May 2020 in 600-bed tertiary care teaching hospital in India having affiliation for both undergraduate and postgraduate medical teaching. A pre-structured questionnaire was randomly distributed to 500 students by sharing a link in the email addresses of the students. Data were entered and analysed in the Microsoft Excel 2010 using descriptive statistics. Results: So far as the different methods of online teaching are concerned, the Interactive online sessions were attended by 59.5% of the study respondents, 28.3% of the study respondents mentioned attending webinars while uploaded lectures were attended by 12.2% study respondents. 150 (57.3%) of the respondents mentioned that the online training sessions were equally effective as in-person training classes. However, a sizeable proportion of 90 (34.3%) respondents mentioned that the online training sessions were less effective. Conclusion: Though technology has been rapidly and innovatively used to ensure medical education services continue unhindered, it has thrown up major challenges. Clinical Experience: Out of 87 surgeries performed, 11 patients; 9 males and 2 female, with no prior history of allergies, developed superficial skin lesions either on the evening of operation or the morning next day. The clinical presentation varied from erythema and superficial desquamation of skin to painful blister formation to sloughing of the whole skin of the genitalia and inner thighs. After ruling out another differential diagnosis, we inferred that 3-4 times concentrated CHG was used in these patients leading to the chemical burns. On Naranjo Adverse Drug Reaction Probability Scale, the probability for chemical burn by CHG was 6. Conclusion: The case reports here described partial-thickness chemical burns in adults after accidental skin preparation with higher than desired concentrations of CHG in alcohol disinfectant. This stresses the need for collective responsibility of the health care team in peri-operative care as well as conduction of regular and thorough audits if any adverse event occurs. Aim and Objective: When a person with Tran Message encounters someone with disease symptoms, that person will automatically touch the affected area, diagnose the unhealthy or affected part of the body and empathize with it. In this study, clients are taught to recognize the physical, psychological, mental, and environmental problems of their health and helped them to practice self-healing by recovering their inherent potential. According to the Diagnostic and Statistical Manual of Mental Disorders DSM5, ASD is characterized by extreme unresponsiveness to others, severe communication deficits, overly rigid and repetitive behaviours, interests, and activities. Narrative research is a way to find the meaning of experiences in the process of talking about and retelling the experiences with which human beings meet in life and to study the process of living a new life based on this. The average age of Alzheimer’s patients is now in the range of 65-80 which was earlier of 75 and above. It has always been challenging to the doctors for proper diagnosis and planning of appropriate treatment of this disease. The manual identification of Alzheimer’s disease from MRI images is influenced by different factors and may vary from experts depending on their expertise in the diagnosis of the disease. There is no efficient technique so far for the accurate diagnosis of Alzheimer’s disease at the very early stage to prevent the progress of the disease. Objectives: The main aim of the study is to develop an algorithm which could detect the presence of Alzheimer’s disease from the brain MRI images by extracting the brain features so that the presence of Alzheimer’s disease can be detected at the initial stage itself and appropriate treatment could be given at the right time to prevent its progression. Methods: About 6000 MRI brain images which are used for the study is obtained from ADNI Database. The MATLAB toolbox is used for design and programming. A new algorithm is developed using combined feature extraction using SURF, FAST, BRISK, Harris and Min Eigen methods followed by HOG and feature selection using Principal Component Analysis method for early prediction of various stages of Alzheimer’s disease. An analysis of the proposed method is done by combining it with k Nearest Neighbor, Decision Tree and Naïve Bayes and Random Forests classifiers and the performance parameters are evaluated. Results: The accuracy of 98.3% is obtained when K-nearest Neighbor is used for classification of Alzheimer’s disease. This is followed by Decision Tree having an accuracy of 98.13%, Naive Bayes with an accuracy of 97.31% and Random Forests having an accuracy of 97.12%. Conclusion: The proposed algorithm is found to be superior to the methods which use only one feature extraction method which is developed for the prediction and classification of Alzheimer’s disease. Wide variety of organisms are implicated in its aetiology, gram-negative bacilli (GNB)being most common. UTI is treated with a variety of broad-spectrum antibiotics. Resistance to antibiotics is increasing nowadays. Gram-negative bacilli confer resistance to βlactam antibiotics by producing enzymes like Extended-spectrum β – lactamase and Amp C β – lactamase. The present study was carried out with the aim and objectives to isolate and identify GNB causing UTI and to prepare antibiogram of these isolates. The study also includes to find out the prevalence of Amp C β – lactamase producing GNB causing UTI. Materials and Methods: The urine samples obtained from patients having suspected urinary tract infection were processed to isolate and identify the causative organism. Antibiotic susceptibility testing was done by Kirby Bauer Disk Diffusion method for gram-negative urinary isolates in patients with significant bacteriuria. Clinical Laboratory Standards Institute (CLSI) guidelines were followed during the interpretation of results. GNB showing resistance to 3rd generation cephalosporins and resistance to cefoxitin were subjected to Amp C Disk Test to detect Amp C β – lactamase producing GNB. Minimum Inhibitory Concentration (MIC) for ceftazidime was tested for these isolates. Results: Total of 288 samples from patients with suspected UTI were processed.214 samples were culture positive. Out of which 167 samples showed significant bacteriuria (78.03%). Gram-negative bacilli isolated were 74.85%. All GNB showed reduced susceptibility to 3rd generation cephalosporins. They were susceptible to amikacin, nitrofurantoin, imipenem, meropenem. Pseudomonas isolates showed resistance to imipenem and meropenem. Prevalence of Amp C β – lactamase producing GNB was 21.6%. Conclusion: Gram-negative bacilli producing UTI were 74.85%. These isolates showed resistance to 3rd generation cephalosporins. They were susceptible to amikacin, nitrofurantoin, imipenem, meropenem. Pseudomonas isolates were resistant to imipenem and meropenem. Prevalence of Amp C β – lactamase producing GNB was 21.6%. Small colony variants(SCV) of staphylococci are regarded as one of the mechanisms for resisting therapy that extends beyond the classic forms of resistance. They are slowgrowing subpopulations of Staphylococcus aureus (S.aureus) which are often associated with persistent or recurrent infections and frequently missed for diagnosis. The present study was undertaken to determine the prevalence of SCV of S.aureus among clinical specimens received from in-patients and out-patients attending attached hospitals of a tertiary care centre. Methodology: The present study was undertaken as a laboratory-based descriptive study, carried out in the Department of Microbiology in a tertiary care hospital over two years from May 2018 to April 2020. After obtaining ethicalclearance for the study, a total of 1000 consecutive S.aureus isolates from different clinical samples routinely received in the laboratory from outpatient and inpatient departments were screened for SCVs. The clinical specimens were processed as per standard microbiological protocols. Phenotypic characters were noted along with auxotrophy and antimicrobial susceptibility. The isolates were also characterized by molecular methods for nuc and mec A genes. Results: 6 isolates of SCV were noted during the study among clinical specimens, 4being HA-MRSA and 2 CA-MRSA. All these isolates were found to be methicillin-resistant. The SCVs exhibited delayed coagulase and heminauxtotrophy. nuc and mec A genes were detected by a polymerase chain reaction and confirmed as methicillin-resistant S.aureus. Conclusion: In the present study, 6 isolates of SCV were noted. Detection of SCV poses a challenge in laboratory practice. The high degree of suspicion and meticulous reporting of SCV aid inappropriate management of cases. It is one of the parasitic infectious diseases that may affect haematological parameters. Most common haematological complications associated with malaria are Anemia, leukocytosis, thrombocytopenia and leucopenia. The magnitudes of these changes vary with endemic malaria, background hemoglobinopathy, nutritional status, demographic factors and immunity from malaria. Aim: The main objective of this study is to evaluate the haematological profile of patients infected with malaria. Material and methods: Total 120 malarial positive patients with the cases of haematological disorders were included in this study. From the patient’s Blood samples referred for peripheral blood, the smear was collected in ethylenediaminetetraacetic acid (EDTA) and were also analyzed for malarial parasites with conventional microscopy. Giemsa stain was used for Peripheral smear and also PCV, Hb and WBCs Total counts, RBC morphology, and WBCs differential count and platelet counts are also done. Result: In this study, the main clinical feature was chill fever. Anaemia was seen in almost all cases of malaria. Leucopenia was seen in 29.17% of cases. The incidence of leucopenia was 29.4% in P. vivax and in P. falciparum was 27.6% and 33.3% in both P. vivax and falciparum. Monocytosis was seen in 48.24% cases of P. vivax and 17.24% cases of P. falciparum and 33.33% of incidence in both P. vivax and falciparum. The significant co-relation between thrombocytopenia and Malaria was found in this study. Conclusion: This study showed that almost all blood components and is a true haematological infectious disease and in P. falciparum substantially higher compared with P. vivax. Those programs must include contents on the perception of well-dying and preparation for well-dying. Aim and Objective: This study helps us to grasp the subjectivity and the characteristics of each type of how Korean elderly perceive good death. By applying the Q methodology, 31 elderly aged 65 or older living in G metropolitan city were selected as a convenient sampling method to classify 28 sentences of well-dying. Data were collected by distributing 28 selected Q samples on a Q distribution on a 9-point scale. Collected data were analyzed by Q factor after making data-file. The collected data scored the conversion points assigned to each of -4 with 1 point, 5 points neutral, and 9 points +4 with cards forcibly distributed on the Q sample distribution table. By coding the given conversion scores in order of Q sample number, Using PQ method. As a result, the well-being perceived by the elderly was recognized as three types of Type1 is ‘accepting death’, Type 2 is ‘preparing death’ and Type3 is ‘peaceful death’. Conclusion: The results of this study will be used as a basis for developing a more in-depth well-dying education program, specific and differentiated well-dying education is needed based on the perception of the death of the elderly and interventions from the perspective of the elderly to prepare for well-dying. Shamana (conservative) and Shodhana (biological purification of the body) treatments are advised in Ayurveda whereas anti-inflammatory, analgesics, steroids, and disease modifying antirheumatic drugs are required for its management as per modern medicine, which is not free from side effects. The symptoms of Amavata are alike to Rheumatism, which include Rheumatoid arthritis and Rheumatic fever. It is observed that Rheumatism is an autoimmune disorder, which is among the collagen disorders having strong and significant parlance with Amavata. A variety of drug trials were already carried out on Amavata, yet there is a lacuna in the management of Amavata. Hence the present study was designed to calculate the effectiveness of Ayurvedic formulations in Amavata. The study was conducted in 15 clinically diagnosed cases of Amavata (Rheumatoid Arthritis) with objectives of Clinical Evaluation of Therapeutic Effect of Rasnadikashayam 3 t.s.f. thrice/day, Sahachradikashayam 3 t.s.f. once/day, Amvatari Ras 2 tabs once/day were given for 3 months and Erand tail ½ t.s.f.once/day at night were given for 1 month along with Swedan therapy in patients of Amavata (Rheumatoid Arthritis) on the basis of various scientific parameters. Patients were treated with the whole therapeutic regime mentioned in Ayurvedic classics i.e., Langhana, Swedana, use of Tikta, Katu, and Deepan dravyaprayoga, Virechan. The effect of the therapy was assessed by a specially prepared proforma and at the end of the treatment, 39.39% improvement was observed in the overall effect of therapy. Paired t-test was applied to this BT and AT scores as these grades is accepted as per international norms. It showed that administered therapy reduced the degree of disease activity significantly as P < 0.001. The current study was done to discover diverse appendicular histopathological lesions, their statistics and the percentage of a ruptured appendix, incidental appendectomy and negative appendectomy. Type of study: Retrospective cross-sectional study. Materials and Methods: Resected samples of appendices were given to histopathology Department of Pathology Chandulal Chandrakar Memorial Medical College and hospital. Results: Out of 1180 samples of the appendix, appendicitis accounted for 90.68% with peak age incidence in the age group of 15 to 25 yrs in both genders. Histopathological diagnoses of different types of appendicitis included acute (49.06%), acute suppurative (22.06%), gangrenous (17.66%), perforated (3.64%), resolving/ recurrent chronic (2.90%), acute eosinophilic (3.46%), xantho-granulomatous (0.84%), and carcinoid tumcurrent (0.37%). Other important accompanying pathologies were parasitic infestation and Meckel’s diverticulosis. Negative appendectomy rate was 7.8% which was much more common in females. Conclusion: Incidence of appendicitis is higher in adolescents and young adults and slightly higher in females. Negative appendectomy is also very common in females. Last but not the least, an unusual finding such as carcinoid tumour in the clinically suspected acute appendicitis reflects the value of histological examination of every resected appendix and its presence would change the currents of further management. Fast spin-echo imaging, used for fat suppression in conjunction with MR technique, has improved the sensitivity and accuracy of MR to detect injuries to the articular cartilage. Besides, three-dimensional (3D) volume techniques have shown MR imaging ‘s flexibility in measuring meniscal tears. This can be used to reformat orthogonal and non-orthogonal representations of meniscal tears planes. Aim: The present study was done for evaluation of meniscal injuries of the knee on MRI. Material: This study was carried out in 100 subjects (63 males and 37 females) ageing from 12 to 73 years. Results: The findings obtained showed that medial meniscal tears/degeneration (64 %) accompanied by anterior cruciate ligament tear (49 %) were the most frequently found pathology of all patients. Conclusion: Magnetic resonance imaging has a high degree of sensitivity, precision and accuracy in the diagnosis of the meniscal, posterior cruciate ligament and full anterior cruciate tears in detecting partial anterior cruciate ligament tears. Each Prakriti has a typical psychosomatic constitution. Prakriti of an individual can be assessed with the help of the characteristics that are described in ancient Ayurveda texts. Assessment of Prakriti can be helpful in determining the stress handling capacity of a person and ultimately helping him to keep stress-induced diseases away. Material and method: Online survey forms of Prakriti and stress handling capacity were distributed to 200 students. Out of 200 students 138 responses were received. Prakriti and stress handling capacity was assessed from the responses received. The data was compared to look for the relationship in them, if any. Conclusion: Kapha and Pitta found to be the most important factors in the psychosomatic composition of a person. People with the dominance of Vata are more prone to stress induced diseases. Health-related quality of life (HRQOL) is an important outcome of health. Any adversity in health such as musculoskeletal disorders (MSD) will affect HRQOL. Modern defence system has significant role of electrical, electronic and Instrument technicians in armed forces. Despite their intensive role, little is known about MSDs & HRQOL in these technicians. Method: 164 Electrical, telecom and instrument technicians were assessed for MSDs and HRQOL, using Nordic Musculoskeletal Questionnaire (NMQ) and RAND 36-Item Health Survey 1.0. All eight domains of HRQOL were compared for personnel having MSDs with those who were unaffected. Effect of MSDs and its symptomatology was assessed on physical component score (PCS), mental component score (MCS) and ‘Well being’ scores of HRQOL using logistic regression. Results: High prevalence of MSDs (73.45%) and negative effect of MSDs on all domains of HRQOL was noted. Though presence of MSDs, multiple MSDs, increase frequency and severity of pain negatively affected PCS, MCS was affected only with increased frequency of pain. MSD, frequency and severity of pain had significant negative impact on Well-being. Conclusion: There is high prevalence of MSDs among light maintenance defense personnel and these MSDs have adverse effect on HRQOL. We suggest regular screening and suitable preventive/therapeutic measures to control the occurrence and its symptoms, thereby maintaining optimum HRQOL. Methods: Current study was done to determine whether spraying 0.5% Levobupivacaine under the surface of diaphragm and gallbladder bladder fossa have any positive result in pain management after surgery. The current study was conducted on a total of 50 patients out of which half (group I) were administered Levobupivacaine and in the remaining half (group II), Levobupivacaine was not used. Results: Numeric Rating Scale of pain at 1, 6, 12 and 24 hours after surgery showed remarkable improvement in NRS scores in group I as compared to group II. Conclusions: Postoperative pain was reduced and the analgesic requirement was delayed up to 12 to 14 hrs in the test group, whereas in the control group analgesia was required within 6 to 8 hrs. According to most International Federation of Gynecology and Obstetrics (FIGO) was devised as the most acceptable system of nomenclature and classification of AUB in the year 2011, namely PALM–COEIN classification to describe AUB in the year 2011. The objective of the present study was to find out causes and to categorize the causes of AUB as per the FIGO system in terms of the structural (PALM) and functional (COEIN) components in the Indian scenario. Method: The present study was done retrospectively carried out on 120 non-pregnant reproductive age women with complaints of AUB at Datta Meghe Medical College, Shalinitai Meghe Hospital and Research Centre, Wanadongari, Nagpur, during the period May 2019 to April 2020. The study aims to conduct the efficiency and practicality of the PALM-COEIN classification system for a patient showing AUB at the outpatient department. Results: It is a retrospective study carried out on 120 patients presenting with complaint of abnormal uterine bleeding. Upon classification of the patients according to the PALM-COEIN classification, Leiomyoma was found to be the most common cause of AUB in patients with gynae OPD (n=38, 31.6%) was followed by ovulatory dysfunction was followed by (n=28, 23.4%), Adenomyosis (n=22, 18.4%), Endometrial causes (n = 13, 10.82%), Polyp (n=8, 6.6%), Not yet classified (n=7, 5.8%), Iatrogenic (n =2, 1.6%), Malignancy and Coagulation (n = 1, 0.83%). About 69 (57.5%) patients had structural (PALM) causes of AUB in our study and 51 (42.5%) are non-structural cause (COEIN). Conclusion: The PALM COEIN classification system of AUB for determining diagnosis of causes and helps us to facilitate proper management of women with a broad group of disorders included under acute and chronic AUB Leiomyoma was found to be the most common cause of AUB in patients with gynae OPD after classification of the patients according to the PALM-COEIN classification. Many researchers used various algorithms and different approaches to predict swine flu. Aim and Objective: In this paper, we propose Naive Bayesian classifier for an efficient prediction, by comparing the symptoms and applying significant signs and prediction factors to Bayes theorem to predict the patient’s condition. Method: Here in this work, the machine learning algorithm is used on the available data to reduce the time and to predict the disease perfectly. We collected 14 sample records of different swine flu suspected patients from King George Hospital, Visakhapatnam, India, based on the sample reports. We prepare the training SF (Swine Flu) dataset. The ESFP (Efficient Swine Flu Prediction) is first trained by using SF (Swine Flu) dataset with some of the significant symptoms observed in Swine Flu cases. Conclusion: The first signs and the prediction values are applied to Naive Bayesian Classifier by using the training set, and then the test set is applied to predict the disease prevailing or not.. JungariEnglish Deepti ShrivastavaEnglishBackground: Pregnancy-induced hypertension (PIH) is a leading cause of high-risk pregnancy and is detrimental to both mother and the child. Hence this study was undertaken to study the effect of magnesium sulfate combined with calcium channel blocker in the management of pregnancy-induced hypertension. Methodology: A total of 120 pregnant women admitted with PIH in the Department of OBGY in a tertiary hospital from October 2019 to April 2020 were randomly divided into the control group (60 cases) and the observation group (60 cases). The observation group was treated by magnesium sulfate along with a calcium channel blocker; i.e. nifedipine, while the control group was treated by only magnesium sulfate. Agarwal is Informative. Hematological profile of patients with various types of malaria is useful to postgraduates of Pathology and Medicine. Histopathological spectrum of appendicular le- sion by R. K. Chandrakar is quite useful for junior residents especially in Pathology. Study of MRI brain images for early diagnosis of Alzheimer’s disease is interesting for the neu-rophysicists. Efficacy of Ayurveda treatment in Rheumatoid arthritis by Wairagade et al. is nicely covered and useful in treatment of chronic cases. It is not possible for me to cover all articles in the note, all are excellent and academic feast for global medical readers, but I am sure that our young researchers and especially postgraduates, doctors would take the maximum benefit out of them. For a high IF, and Pubmed we need to have more of good quality original articles with high citation values. All senior members are urged to submit their prime research work and consider IJCRR as their first-choice journal. Further, suggestions of any type, especially related to any columns to be included, are most welcome. The articles will help postgraduates in their outlook to problematic cases. My sincere appeal to authors that though we have achieved SCOPUS indexing it is just a beginning. We have to keep up our moto of journal for Pubmed/Medline with skillful execution, dedication hard work and increasing our citations. In this COVID – 19 pandemic , stay home stay safe take care’’.
